Uninsured services

We provide several medical, administrative, and documentation-related services that are not covered by your Ontario Health Insurance Plan (OHIP). Below is an overview of these services to help you better understand what might incur additional charges.

Medical Services

We offer the following medical services not covered by OHIP:

  • Cosmetic Procedures: Removal of warts, age spots, or skin tags.

  • Travel Medicine Advice: Consultation and services for international travel health needs.

  • Non-Routine Examinations: Includes diagnostic tests or exams requested by employers, insurance companies, or government agencies.

  • Pre-Adoption Examinations: Specialized medical evaluations required for adoption.

  • Prescription Renewals: For patients who are not part of our roster.

  • Routine Office Visits: Visits without a valid OHIP card.

Forms and Medical Records

We assist with completing various forms and managing medical records, including:

  • Work and School Forms: Includes sick notes, back-to-work notes, and admission forms for daycare, schools, or recreational programs.

  • Medical/Legal Reports: Preparation of documents for legal or insurance purposes.

  • Government Forms: Completion of federal government documents.

  • Certificates: Issuance of certificates for travel, employment, or other purposes.

  • Driver’s and Aviation Medical Exams: Includes forms required for licensing.

  • Insurance Forms: Such as disability certificates, treatment plans, and medical examination reports.

Administrative Services

We also provide administrative support for your medical documentation needs:

  • Medical Record Management: Copying, transferring, or summarizing medical records upon patient request.

  • Missed Appointments: Charges may apply for missed appointments without sufficient notice.

Renewals

Renewals of prescriptions and service recommendations may also incur charges:

  • Prescription Renewals: Processed through pharmacy requests.

  • Service Renewal Requests: For Registered Massage Therapy, Physiotherapy, Counseling, or other complementary services.
